High Performance Support Fund Application

Application form

Applications for the High Performance Support Fund are available to High Performance Support Programme students who have been selected to represent New Zealand or New Zealand Universities in domestic and international competitions or events and need financial support attending. Non New Zealand representation applications may in some instances be granted funding. Grants will be provided to assist with travel, accommodation, registration or equipment costs. The demand for grants usually exceeds the financial resources available so it is unlikely all applications will be successful. Applications are assessed and prioritised by a Support Fund Panel.

To be eligible, students must:
• Be a member of the High Performance Support Programme
• Upload an unofficial academic transcript (see how to get one here)
• Upload a budget, quotes and/or receipts (flights or paid invoices)
• Upload a bank deposit slip or bank statement (an exemplar can be found here)
• Upload a high quality photo of themselves, preferably in action
• Not already be a recipient of a grant from this fund this academic year
• Provide supporting documentation from your verified national body confirming selection or competition

Please enter a dollar value. Funding is usually up to a maximum of $1000 per student.

Who is hosting and/or supporting this event? World and/or National Sporting Organisation etc.

How will the University benefit or be acknowledged for the support? Will there be opportunities for media releases or communication to the University community post-event?

Have you applied to or are receiving funding from any other organisation for the same purpose?

If so, please provide further details. Name the funding body, amount requested and date of when decision will be reached

Upload budget, quotes and/or receipts. These should be from your National Body on their letterhead or a receipt or invoice in your name with the name of your selected competition stated.

Upload bank deposit slip or statement (must include bank number, bank logo, your name and be no more six months old.)

Declaration

I declare that all information submitted is true and correct and that no relevant information has been withheld. If successful I agree to provide receipts, a written report (including confirmation of students attending competition) and photos/film footage to the panel. I acknowledge that failure to properly account for grant funding will result in the University seeking reimbursement for all unaccounted funds and will jeopardise future funding application. I agree that the University of Auckland may access my academic transcript if not provided and publish articles, photos and/or film of this project in its internal and external communications.
